The Decision-Making & Judgement system integrates all other systems. It determines how signals are interpreted, how options are assessed, and when action is taken. When it is functioning, system strain is identified early and managed effectively. When it weakens, every other system is left to degrade without intervention.

Decision failure is not a single poor choice. It is a gradual narrowing of awareness under fatigue, pressure, and cognitive bias. By the time a clearly poor decision is visible, the system has often been degrading for some time.
